DESCRIPTION OF SERVICES

Heart2Care Home Care has engaged qualified RNs, LPNs, RPNs, and HCAs to meet the diverse needs of our clients.

Registered Nurses (RNs):

  • Comprehensive health assessments
  • Medication administration and management
  • Wound care and dressing changes
  • IV therapy
  • Patient education and counseling
  • Care coordination and collaboration with other healthcare professionals
  • Monitoring vital signs and overall patient condition

Licensed Practical Nurses (LPNs):

  • Administering medications (under RN supervision)
  • Primary wound care and dressing changes
  • Monitoring patient health status
  • Assisting with activities of daily living (ADLs)
  • Providing basic patient education
  • Collecting samples for testing

Registered Psychiatric Nurses (RPNs)

  • Mental health assessments
  • Psychiatric medication administration and management
  • Individual and group therapy sessions
  • Crisis intervention and de-escalation techniques
  • Collaboration with psychiatrists and other mental health professionals
  • Education and support for patients and families regarding mental health conditions

Health Care Aides (HCAs):

  • Assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
  • Mobility assistance and transfers
  • Light housekeeping and meal preparation
  • Providing companionship and emotional support
  • Monitoring and reporting changes in patient condition to nursing staff
  • Assisting with a range of motion exercises as directed
  • Running errands and accompanying patients to appointments
